    • Certificate of Incorporation (for Private Ltd / OPC / LLP)
    • Registered Partnership Deed (for Partnership Firms)
    • GST Registration Certificate or Shops & Establishment License (for Sole Proprietorships)
  • Memorandum & Articles of Association (MoA & AoA) / LLP Agreement: To prove the constitution of the business entity.

2. Proof of Address of the Business

You must submit official proof showing the active business location:

  • Lease Deed or Rent Agreement, along with the latest utility bill (Electricity bill, telephone bill, or water bill) in the name of the premises.
  • A No Objection Certificate (NOC) from the property owner if rented.

3. Promoter / Director Details

Provide the following for all directors, partners, or the proprietor:

  • PAN Cards and Aadhaar Cards
  • Digital Signature Certificate (DSC) of the authorized signatory
  • Contact details (Email ID and Mobile Number)

4. Employee Information

You must provide specific details about the employees working in your establishment:

  • Total employee count, along with date of joining and individual salary structures.
  • List of employees earning less than ₹21,000 per month (for ESI threshold tracking).
  • List of employees earning less than ₹15,000 per month (for EPF threshold tracking).
  • Bank details (IFS Code, Account Number) and Aadhaar cards of the employees.

5. Financial & Banking Proofs

  • A cancelled cheque from the company's active current bank account.
  • First page of the bank passbook or statement showing the company's name and address.
